Plains Medical Center has been awarded $35,000 by Health
Resources and Services Administration (HRSA) to assist in the
adoption of a Patient-Centered Medical Home model.
"A Patient-Centered Medical Home is a model of care that
strengthens the clinician-patient relationship by replacing
episodic care with coordinated care and a long-term healing
relationship," according to Morgan Honea, CEO/President for Plains
Medical Center. "Each patient has a relationship with a primary
care clinician who leads a team that takes collective
responsibility for patient care. PMC is excited to be at the
forefront of the movement toward more personalized, coordinated,
effective and efficient care."
PMC is a Federally Qualified Healthcare Center providing
comprehensive primary medical, dental and mental health services
